Dr Sanjeev Nayak - Saving Lives of Stroke Patients

Dr. Nayak is a Consultant Interventional Neuroradiologist at University Hospitals of North Midlands, who introduced and pioneered Mechanical Thrombectomy for stroke patients in the UK. After obtaining Neuroradiology fellowship from the Neurosciences Centre at Newcastle-Upon-Tyne, he did a post-fellowship training in mechanical thrombectomy in Austria. He brought back the skills he acquired in Europe and helped set up the Mechanical Thrombectomy service in Staffordshire that has become one of the leading centres of excellence for minimally invasive Stroke thrombectomy in the UK.

Evidence: The previous conventional treatment of intra-venous thrombolysis in severe disabling large vessel occlusive strokes was effective in only 15-20% of patients and such patients had a prolonged hospital stay of 60-90 days costing hospitals £31500 per patient and with millions of pounds in burden to social care. Dr Nayak introduced a pioneering mechanical clot extracting interventional technique and produced the largest patient series (n=440) treated with this technique in the UK with a mortality rate of 17% (top 3 in UK) compared to 50%1-year mortality previously. The median hospitals stay for patients undergoing MT is now 14 days, less than a sixth of the previous figure of 60-90 days. More than 90% of patients are discharged home, compared to previous situation when over 70% would go to a nursing home because of significant impairment.

National Impact: A National Institute for Health and Care Excellence (NICE) analysis published in March 2016 based on data from Dr Nayak’s service, showed offering the procedure could achieve savings of £2.4 million a year as a result of reduced time in hospital and savings from on-going social care costs. If extrapolated to rest of the UK, a £440 million annual savings can be generated. This new treatment has now been adopted by the Department of Health (April 2017) to be rolled out to the rest of the NHS. Dr. Nayak is also an invited faculty to many national and international conferences and is considered one of the 'world experts' in mechanical thrombectomy for stroke.

Awards: Dr Nayak has been recognized by his peers and his patients for his outstanding achievement in establishing the service which has expanded under his leadership to cover a wide region from Midlands through to North Wales. Having been nominated for 13 national awards (2012-2018), in various categories of BMJ and HSJ awards including “NHS Inspirational Leader of the Year 2013” NHS Leadership recognition awards and “Clinical Leader of the Year 2012” HSJ award. His most recent national awards include:
1)     The Sun newspaper’s “Who Cares Wins Award” for “Ground-Breaking Pioneer or Discovery Award” after he was nominated by a stroke patient.
2)     The highly prestigious Health Service Journal (HSJ) Award for “Specialist Services Redesign”

The mechanical thrombectomy service for stroke has saved the lives of many patients who would have otherwise died or sustained permanent neurological deficits. In addition to the lives saved, this service has resulted in cost saving to the NHS that runs into millions of pounds, by preventing or minimizing stroke disabilities.

A revolutionary change for patients suffering from large vessel occlusive strokes. a great step forward for the betterment of mankind. The kind of data emanating from the aftermath of this procedure is mind boggling in terms of hospital stay, cost and mortality rates. Going forward we can see this getting to zero mortality rates and further reduction in cost in procedures.
